The Cook County Department of Health (CCDPH) offers in-home COVID-19 vaccination services to any individual 5 years and older in suburban Cook County (1st, 2nd, and 3rd doses and boosters). To register, complete the form found here.

Individuals living outside suburban Cook County are not eligible for this program. Individuals in Chicago, Evanston, Oak Park, Skokie, and Stickney are currently not eligible for this program as they may provide these services through their own certified health departments.
After submitting the form, a confirmation email will be sent to the email you entered and if you meet the eligibility requirements, you will be contacted by phone by a In-Home Vaccination Team to schedule your in-home vaccination. Additionally, if you know someone who fits these criteria, please forward this form to them, or if needed complete it on their behalf.
