New Cook County COVID-19 Mitigations: Effective January 3, 2022
On December 23, the Cook County Department of Public Health issued new COVID-19 mitigations that will go into effect on Monday January 3, 2022. Patrons of certain businesses must now show proof of vaccination when entering indoor dining establishments with on-premise dining (restaurants, country clubs, coffee shops), event spaces, theaters and fitness centers. These restrictions do not apply for food carry out. Proof of vaccination status should be with one of the following:
- A CDC COVID-19 Vaccination Record Card; or
- An official immunization record from the jurisdiction, state, or country where the vaccine was administered
- An ‘app’ that provides a digital record of vaccinations
- A digital or physical photo of such a card or record that includes the individual’s name, the vaccine brand administered, and the date the vaccination was administered.
